As of January 2019, not only is your insurance provider required to cover your vasectomy in full, co-pays and cost sharing are no longer allowed for most plans. We recommend you check with your insurance provider if your insurance falls in this category.

If you have an HSA/HRA insurance plan, you may not be entirely covered. If your insurance plan is federal, it may not be subject to state laws. Your insurance needs to be fully funded for it to be covered. There are always some implementation questions, as with any new law. As of now, some HSA/HRA plans may require some payment.
